Definition

Internal report into a marine incident.

An incident investigation report is the internal company document recording the facts, analysis, and corrective actions following an accident, hazardous occurrence, or near-miss. ISM Code section 9 requires the SMS to ensure such events are reported, investigated, and analyzed to improve safety and pollution prevention, with corrective action under section 9.2 to prevent recurrence. The report establishes the sequence of events, identifies immediate and root causes, and assigns preventive measures with owners and deadlines. It is distinct from a flag state marine safety investigation under the Casualty Investigation Code. Reports are reviewed by the DPA and audited internally and by class.
