Berth to berth voyage plan under SOLAS Chapter V.
The berth-to-berth voyage plan SOLAS Chapter V Regulation 34 requires for every voyage, built in four stages: appraisal of all relevant charts and publications, detailed planning of the track, execution, and continuous monitoring against the plan. It covers safe water, no-go areas, contingency anchorages, under-keel clearance, and reporting points, and is approved by the master before departure. A defective or absent passage plan is a common root cause cited in grounding investigations.
