Definition

Documented evaluation of hazards.

A risk assessment is the documented process of identifying hazards, evaluating the likelihood and severity of harm, and applying controls to reduce risk to as low as reasonably practicable. ISM Code section 1.2.2.2 requires the company to assess all identified risks to ships, personnel, and the environment and establish appropriate safeguards. Onboard risk assessments cover routine and non-routine tasks, feed permit to work and job hazard analysis, and are reviewed when conditions, equipment, or personnel change. The register is a primary internal audit and vetting inspection item, and gaps are recorded as non-conformities for corrective action.
