The Casualty Investigation Code is the IMO Code of International Standards and Recommended Practices for a Safety Investigation into a Marine Casualty or Marine Incident, adopted by resolution MSC.255(84) and made mandatory through SOLAS regulation XI-1/6, effective 1 January 2010. Parts I and II are mandatory; Part III is guidance. The Code requires a flag State to conduct a marine safety investigation into every very serious marine casualty, defined as one involving total loss of the ship, a death, or severe damage to the environment. Investigations aim to prevent recurrence, not to apportion blame or liability. Final reports are submitted to the IMO and published in the GISIS casualty module.
