Lockout tagout discipline for machinery isolation.
LOTO, lock-out tag-out, is the safety discipline for isolating an energy source so machinery cannot start during maintenance. The isolating device, an electrical breaker, valve, or switch, is locked in the safe position and tagged with the name of the person who applied it, and only that person removes it. LOTO is a core control in shipboard permit to work and job hazard analysis for work on pumps, main and auxiliary engines, steering gear, and electrical systems. It sits within the SMS under ISM Code section 7, which requires procedures and instructions for key shipboard operations. Failure to isolate is a leading cause of crush and electrocution casualties.
